3. 本人已詳細閱讀備註及聲明，徹底了解其內容，並同意提供個人資料，且基於自由意志於網上繳交此報名表。
4. 如中、英文兩個版本有任何抵觸或不相符之處，應以中文版本為準。

2. I, hereby declare that all the personal information provided for registration is accurate and correct. If any false information is found, MR.J Sports and Entertainment, MR.J Sports Association of Macao, the venue where the training camp take place are not attached legal responsibility for related accidents and issues and reserve the right to pursue any accountability.
3. I, hereby declare that I have read and understood the declaration, and agree to provide my personal information, and that I submit the application in my own free will.
4. If there is any conflict or inconsistency between the Chinese version and the English version, the Chinese version shall prevail.
肖像權同意書:
本人同意並授權拍攝方(賈立賢體育及娛樂活動策劃有限公司)在訓練營或活動舉辦期間拍攝照片、影片及錄音、其後進行修飾、使用、公開展示本人之肖像於各種形式的著作載體及媒體，並可於日後為相關教育、展示、廣告、宣傳、出版等使用。本人將不會對以上材料的以上用途索取任何報酬並同意澳門賈立賢體育會及賈立賢體育及娛樂活動策劃有限公司擁有以上照片、影片、音頻材料(內含上述授權之肖像)及其所有衍生品的獨家使用權及完整之著作權。
*本授權書的簽署表明知悉以上所有條款並無異議。
The Agreement of right of publicity
I hereby agree and consent to the following:
MR.J Sports and Entertainment Events Planning Company Limited has the rights to take photographs or videos, record audio during the Training Camp and to use, copy, modify, reproduce, adapt, distribute, publish, display, exhibit the photographs and/or videos for commercial and business purposes in any educational, advertising, marketing and promotional materials. I further acknowledge that I will not be compensated for these uses, and that MR.J Sports Association of Macao and MR.J Sports and Entertainment Events Planning Company Limited exclusively own all rights to the images, videos and recordings and to any derivative works created from them.
*The signature of the agreement indicates I have read and understand the above terms and conditions without objection.

* 未滿18歲的參加者由家長 / 監護人作出聲明及簽署
* Once the applicants register for the event/training camp, they will be deemed to have agreed to the above declaration and the agreement of right of publicity.
